Why Was This Plan Developed?
Dr. Diamond offers an affordable, quality dental health plan as an alternative to traditional insurance and we recognize that it can be very difficult for businesses to provide their employees with dental insurance. By eliminating dues involved with insurance companies we are able to lower the cost to each patient while maintaining the highest quality treatment. This plan will allow businesses in our area to offer their employees and families a dental plan in a private practice setting.
What Does This Plan Include?
The Diamond Dental Plan includes:
An Initial Exam
Comprehensive dental exam
Complete periodontal evaluation including periodontal probing and tooth mobility charting
Oral Cancer Screening
Head and neck exam
Treatment plan presentation
Cavity Detector X-rays, if necessary
Prophylaxis (cleaning) including Oral Hygiene Instruction
Six-Month Continuing Care Prophylaxis
Periodic Exam at Prophylaxis
Fluoride Treatment provided for children 19 years of age and under at each cleaning appointment!
Other dental procedures are available at reduced fees

Plan Guidelines
Services and fees are guaranteed for one year. Initial fee is non-refundable. Our plan is for you and is non-assignable to another person.
There is a one year time limit for completion of all services provided by the plan.
It is the patient’s responsibility to make sure they schedule and show up for their appointments.
There is a $45 free for a broken appointment without giving 24-hour advanced notice.
Payment is due at the time the service is rendered.
